Patient Portal
For new users:
  1. Go to www.myeyecarerecords.com.
  2. Click the "Register" button.
  3. Fill out the registration form. The Initial Password is 1234.
  4. Click "Submit Registration" button.
  5. You should next see a pop-up notification that you have successfully registered.
For returning users:
  1. Enter the email address and new password used for registration then click "Log In".
  2. Click "View Document" to view visit information.
  3. Click "Send Message to Doctor" to send a message to the doctor.
  4. Click "LOGOUT" when finished.
